NEW: Our practice uses TherapyAppointment 2.0. Patients have access to a portal to see account information.
Changes you will notice:
- Receipts and superbills will no longer be emailed to patients, but can be accessed in REAL TIME on the portal.
- Text reminders will come to you 3 days prior to the appointment. 48 hour notice for cancellation is requested to avoid missed appointment fees.
- You can send emails and attachments directly through the portal.
There are many new features are exploring with the system. Patient intake forms (links below) are still required for all new and returning patients.
Please read office policies carefully. Commitment to scheduled appointments are important to the patient as well as all other patients on the calendar. Penalties for late cancellations and missed appointments will be enforced due to the increase in demand for mental health services. Thank you for your understanding and consideration.

CHECKLIST TO BRING TO APPOINTMENT
- Appropriate Intake forms (below)
- Report cards, K - current grade level or home school sample work
- Copies of previous evaluations, testing, FIE (shows tests and scores), 504, IEP and ARD plans (if applicable)
- Insurance identification card
- Form of payment
Providing the above records is necessary in order to provide the best clinical assessment.
